SmileHealth Room rules

All medications must be brought in by an adult, the medication must be in it's original container, and the Authorization for Medication Form must be complete; signed by the PHYSICIAN and the parent.

Student's must be fever free, (below 100 degrees F), for 24 hoursbefore they can return to school.

General Information

All out of state students and all 6th grade students must have a Physical Exam completed.

All out of state students and all 7th grade students must have a Dental Exam completed.

All students will require 2 Varicella vaccines by 2010-2011 if they have not had the Chicken Pox disease. Documentation must be sent in to the School Nurse.
